That looks like an old school Red Octane metal pad.
I have one just like this and it matches the 6 arrow pinout Rudeshadow posted. Is there a difference with the pinout from the less old school Red Octane pads?
I have a ddrgame.com pad that has a male vga type connection and what I think is red octane with female type vga and a totally different pinout.
I'm trying to add support to my controller encoder with multiple output and am wondering if I should add another option for newer red octane. If you can't tell, I'm super casual in DDR and play because it is super fun exercise.
